To provide effective case management and program planning to clients who are eligible for services provided by the Women and Youth Center, Inc. (hereafter "the WAY Center"), as well as appropriate referrals when necessary; manage case plans for program participants; and assist the Program Director.

Case Manager – Women and Youth Center
Summary: The WAY Center is a Christian organization that provides spiritual and social services for families in need or crisis. The WAY Center was established to be a place of hope, healing, and transformation for people who are lost and in need of hope: the hungry, homeless, hopeless, and downtrodden. The primary goal is to share the gospel at every opportunity and inspire growth and faith in Jesus Christ to all who will listen.
Position Objective: To provide effective case management and program planning to clients who are eligible for services provided by the Women and Youth Center, Inc. (hereafter "the WAY Center"), as well as appropriate referrals when necessary; manage case plans for program participants; and assist the Program Director.
Classification: Hourly –Full time (30 hour minimum – 4 days a week)
Position Reports to: The Program Director of the WAY Center.
Salary: starting at $15.00 an hour
Essential Duties and Responsibilities: Provide holistic and comprehensive case management services to WAY Center clients including, but not limited to intake assessment, benefit assessment, goal setting, case plan development, progress monitoring, individual money management, advocacy and referrals. Specifically:
· Maintain an average case load of three (3) to five (5) long-term program clients.
· Develop sound assessment of new client needs and qualification for services during intake process, ensure proper forms are completed and client receives follow up care as necessary, and ensure client information is added to the database.
· Establish case plans with New Program Participants (within two (2) weeks of orientation).
· Schedule and maintain a minimum of one (2) follow up appointments per month with active clients for case review and properly record session in client file.
· Schedule appointments for counseling and any assessments needed.
· Call clients participating in mentoring programs each week regarding FOCUS Group, Jobs For Life and any workshops available, provide notifications of any changes and/or arrange transport if needed.
· Conduct client exit interview when client is eligible for completion of the enrolled Program and complete Summary report on all program participants within two (2) weeks of client's completion of the Program or when client becomes inactive.
· Answer phones and direct visitors and clients when the receptionist is not present.
· Properly and promptly, document all client encounters and maintain accurate and complete records of client activity in the service report and client files. Ensure that all documents submitted on behalf of a client are accurate and valid.
· Be knowledgeable of WAY Center Programs, community resources; connect clients with needed and available community resources. Follow-up with clients and agencies, as appropriate, to document use/success of referral.
· Assist and record various client services (i.e., RC purchases, clothing vouchers, Shoppe vouchers, referrals, distribution of bus passes, gas cards, food, diapers, etc.)
· Assist Program Director with various program needs (mentor training, preparing group material, etc.).
· Assist Executive Director in training of office volunteers and organizational needs of office.
· Assist with monthly client activity reports for Board meetings.
· Schedule volunteers and community service workers for work in the WAY Center thrift shoppe.
· Accurately complete time sheets for CS workers and CSP participants. Fax/E-mail time sheets to appropriate agency or provide copies as needed.
Required Qualifications, Skills, and Abilities:
· Strong Christian-based faith and beliefs and regularly attend a local Bible believing church.
· Adhere to Christian maturity, principles, and values (esp. Matthew 18 principle).
· Possess a caring personality, emotional maturity, the ability to be non-judgmental or critical of individuals and the situations, and the ability to be culturally sensitive.
· Ability to maintain a courteous and professional relationship with clients and partnering agencies.
· Knowledge of community resources.
· Experience working with people in crisis.
· Basic computer and keyboarding skills.
· Exceptional organizational and time management skills.
· Excellent documentation skills.
· Effective written and verbal communication skills, ability to establish rapport with clients, fellow employees, other agencies, volunteers, and ministry partners/donors.
· Ability to work in a fast-paced, emotionally charged environment.
· Ability to motivate others toward achieving goals.
· Ability to work independently with a strong sense of focus and a clear sense of boundaries.
· A strong sense of and respect for confidentiality involving both clients, fellow employees, and volunteers.
· Ability to legally operate a motor vehicle and provide own transportation.
Education/Experience Requirements:
· High School Diploma required, AA degree or equivalent experience/ training preferred.
· Proficiency in MS Office
· Knowledge and experience with office equipment (phone, copier, fax, etc...)
· Prior experience in ministry and/or managing client cases preferred.
· Bachelor's degree in social work, psychology, or a related discipline preferred.
